PLAN ACTIVATION - INFORMATION 



1.  Upon purchasing a plan your membership booklet will be mailed to your address provided on the application. You should receive your booklet in approximately 3 - 5 days.


2.  Your membership booklet will explain to you how to activate your Member Benefit Services.


3.  If you have any questions regarding your enrollment please call the Toll Free Membership Loyalty Line 

number found inside your membership booklet.
